ASSOCIATION OF SERUM COPEPTIN, CARDIAC TROPONIN T, AND ADIPONECTIN WITH ANGIOGRAPHIC SEVERITY OF CORONARY ARTERY DISEASE: A HOSPITAL-BASED CASE-CONTROL STUDY

Abstract

Background: Copeptin reflects activation of the arginine vasopressin stress axis, cardiac troponin T indicates myocardial injury, and adiponectin has anti-inflammatory and anti-atherogenic properties. Their combined relationship with angiographic coronary artery disease (CAD) severity remains incompletely characterized. Objective: To compare serum copeptin, cardiac troponin T, and adiponectin concentrations between patients with angiographically confirmed CAD and controls and to examine their relationship with angiographic disease severity. Methods: This hospital-based case-control study included 160 participants: 80 patients with angiographically confirmed CAD and 80 controls. Serum copeptin and adiponectin were measured by enzyme-linked immunosorbent assay, and cardiac troponin T was measured by chemiluminescence immunoassay. CAD severity was assessed using the Gensini score. Group comparisons, correlation analyses, and receiver operating characteristic (ROC) analyses were performed. Results: Among the CAD cases, 11 (13.75%) had mild, 24 (30.0%) moderate, and 45 (56.25%) severe disease. Mean serum copeptin (4.12 ± 2.23 vs 0.62 ± 0.036) and cardiac troponin T (3.36 ± 2.38 vs 0.022 ± 0.01) were higher in cases than controls, whereas adiponectin was lower (6.84 ± 2.16 vs 10.92 ± 3.48); the reported p value for each comparison was 0.001. Copeptin and troponin T showed positive associations with CAD severity, while adiponectin showed an inverse association. Reported AUC values were 0.703 for copeptin, 0.987 for troponin T, and 0.812 for adiponectin. Conclusion: Higher copeptin and cardiac troponin T concentrations and lower adiponectin concentrations were observed in patients with CAD and were associated with greater angiographic severity. Cardiac troponin T showed the strongest discrimination in this study sample. The findings should be interpreted in view of the baseline differences between cases and controls and the observational design.
